Heart & Soul of NM, offers our qualified clients Comprehensive Community Support Services. These services assist clients to access resources and assistance in a number of ways, including areas of work, independent living, recreation, socialization, and learning. Our case workers, or Community Support Workers, are skilled in accessing community resources and assisting with life skills and other needs. These services can take place virtually, in-home, in office, work, or in the community.

Who qualifies for services?

CCSS is a medicaid-paid service. In addition, if an individual's mental health is impacting their ability to succeed independently, in work, in school, socially, or experience joy- we are here to help!

What does a worker do?

Meetings can occur as often as needed depending on the circumstances. You and work worker will establish this early on in your relationship.

How often do we meet?

Meeting frequency is tailored to your needs. Some clients benefit from weekly sessions while others need less frequent contact. We work with you to find the right schedule.

Can services be virtual?

Services can be delivered virtually at times. However, this service is intended to be community-based and in-person is mandatory. This is why we can only deliver these services in Valencia, Bernalillo, Sandoval, or Dona Ana Counties right now vs across the state like our other services.

How do I get started?

Register online on our home page and you will be scheduled for an initial assessment to determine if CCSS is a right fit for you.

If my provider is not a good fit, may I request another?

Absolutely. We believe progress can only be made in relationship. If your initial provider is not a good fit, just send an email to our Executive Director, Brandy Castillo, with the reason for the request and a change and she will assist brandy@heartsoulnm.com
